The U.S. Olympic and Paralympic Movement is committed to an environment free of emotional, physical and sexual abuse. 

This form is to be used to report to the U.S. Olympic Committee an alleged violation of the SafeSport Code as promulgated by the U.S. Center for SafeSport.

As the U.S. Center for SafeSport has exclusive jurisdiction for the response and resolution of sexual abuse allegations, any report of a sexual nature will be reported by the USOC to the U.S. Center for SafeSport.  You can also report directly to them via their online reporting form found here.  An allegation of emotional or physical abuse may be directed by the USOC to the respective NGB for appropriate response and resolution.

INVOLVED PARTIES

Please list the individuals involved including as many of the listed fields as you can provide.

The Claimant is the person who is alleged to have experienced conduct that constitutes a Code violation.
The Respondent is the person who is alleged to have violated the Code.
Third Party Reporter is a person who reports a possible violation of the Code, that is not the Claimant or Respondent.

QUESTIONS




If you are a Covered Adult (an individual over the age of 18 who is subject to SafeSport policies), you are also required to report all instances of possible child abuse and/or neglect to appropriate law enforcement. If you are uncertain whether you are a Covered Adult, review the policies and procedures at www.safesport.org or check with your sport National Governing Body.
