For Coaches Attending the Clinic ONLY:
(To reduce your time in the check-in line when you arrive at the OTC, you can fill out these forms and email (indoorhp@usav.org) or fax them (719-228-6899) by January 25, 2013. No late forms will be accepted and will need to be filled out/brought when you arrive.)
(Travel from the Colorado Springs Airport to the Olympic Training Center is provided for coaches. In order to qualify for this transportation option, coaches must complete the travel itinerary link above by January 25, 2013. Those failing to provide their itinerary are responsible for their own transportation.)
(If you are not a current USAV member, you will need to complete this form. For U.S. citizens ONLY.)
