For Tryouts

The forms are needed for all athletes attending a USAV HP Tryout.

For National Training & Skills Programs

The following forms are required for any USAV High Performance Program. Athletes need to bring signed copies of these forms with them to the program. Please do not fax or mail them in prior to the start of the program. Athletes will not be able to participate without these forms in our possession.

U.S. Olympic Training Center Forms

Required for any athlete participating in a training program on at a U.S. Olympic Training Center, completed online. (link sent by program lead in email)

U.S. National Team Program Forms

The following are required for athletes participating on one of the following (training teams for international competitive events):
U.S. Women's Junior National Training Team
U.S. Men's Junior National Training Team
U.S. Girls' Youth National Training Team
U.S. Boys' Youth National Training Team

Indoor High Performance Cancellation & Transfer Policies

High Performance Tryout Transfer & Cancellation Policy

Families may request a transfer or cancellation for a completed tryout registration at any time up until the start time for the event that you are registered for. Cancellations requested in advance of a registered event will be refunded in full less a $20 administrative processing fee within 1-2 weeks of request submission date. Any questions can be sent to

To Transfer a Tryout Registration:

To transfer from one tryout date and/or location to another, complete the following steps:

  1. Login to your Ex3 account:
  2. Select “My Options” in the left hand column then “MY USAV Events”.
  3. Click on the tryout that you are currently registered for, that you would like to transfer from to highlight it. Once the tryout is highlighted, click the “Transfer Tryout” button.
  4. Click “Ok” in the pop-up window and then follow the registration steps for the new tryout that you are transferring to.
  5. You will receive a new confirmation email for the tryout that you have transferred to. You are now registered for the new tryout date and location.

To Request a Tryout Cancellation (Refund):

To cancel your tryout registration, please complete the following steps:

  1. Login to Ex3:
  2. On the left menu, under “My Options”, select “My USAV Events”.
  3. Select the tryout that you have registered for, and then select “Cancel Tryout”.
  4. Review the details of the transaction that you wish to cancel, and confirm the cancellation request.

Your tryout registration has now been cancelled and your refund will be processed within 1-2 weeks.

*Please note that “No Shows” (athletes who do not show up for an event that they are registered for) are not eligible for a refund or transfer to a different date or location unless the request for cancellation or transfer was completed in advance of the start time of the event that they registered for. If you do not show up for an event that you are registered for and wish to participate in a tryout at a different date or location, you will be required to complete a new tryout registration.

High Performance National Training Program Cancellation Policy

Athletes who are registered for a National Training or Skills Program are eligible for a full refund less a $50 administrative processing fee only if requested within 2 weeks (14 days) from the date that the program invitation is posted to the athlete’s Ex3 account. Cancellation requests after this time period are ineligible for a refund. Please note that all athletes are required to acknowledge and agree to the refund policy prior to completing the registration process.

REGION HP: Athletes who have registered for a USAV HP Training Program at the A2 Level or below who subsequently are invited to a Region HP tryout or training camp that conflicts with the USA HP event will be given consideration for a full refund minus $50.00 processing fee. All cancellation requests on behalf of Region HP athletes must be submitted in writing as soon as possible or within 24 hours following selection to a team.

To request a cancellation, please send an email to including the following information in the subject line of the email: Cancellation - Program Name (ex: Girls' A3 Program - Colorado Springs), and Athlete Name. All cancellation requests must be made in writing via email.
