USA Volleyball

Required Forms (USA and Non-USA Teams)

• 2013 Player Medical Release Form with Notary (FL State regulation, required of all U.S. teams)
• 2013 Player Medical Release Form without Notary (Foreign Teams only*)

*Note that without a notarized medical release form it is possible that medical attention maybe denied by emergency personnel or hospitals for any injured player.  USA Volleyball is not responsible for any denied medical care or claims.

Additional Required Forms (Non-USA Teams Only)

• 2013 Foreign Team Participation Summary Form (1 per Team)
• 2013 Foreign Participant Registration Form (1 for each individual on the roster--staff and players)
• Copy of birth certificate, passport or other legal proof of age (1 per player)
• Copy of photo ID (1 for each individual on the roster--staff and players); i.e. Passport
• A letter of good standing from your national volleyball federation with your team entry and check.
