When is the open enrollment period?

August 1st – October 1st

If I have already affiliated with a club, do I need to do this annually?

No, if your affiliation is staying the same, you do not need to do anything.

Are new members to USA Field Hockey allowed to affiliate themselves with a club post October 1st?

Yes, new members will be able to affiliate with a club once they purchase a membership.

Which events are affected by the Club Affiliation Policy?

Disney Field Hockey Showcase, Regional Club Championships, National Club Championship, and the National Hockey Festival

Which events are not affected by the Club Affiliation Policy?

National Indoor Tournament Qualifiers, National Indoor Tournament and the Disney Jr. Field Hockey Showcase.

Which events are clubs allowed 3 guest players per roster?

Disney Field Hockey Showcase and the National Hockey Festival.

Which events are clubs not allowed 3 guest players per roster?

Regional Club Championships and National Club Championships; all players must be affiliated with the club at these two events.

How do I change my club affiliation during the open enrollment period?

Please log into your account, click on “Change My Club Affiliation”, select the state in which you play, find your club, and click submit. A confirmation window will appear.

If I have an extenuating circumstance, how do I change my club affiliation outside of the open enrollment period?

Please fill out a Club Affiliation Transfer Request form and send it to the National Office by mail, email or fax. The form MUST be submitted 7 days before a roster is due for a USA Field Hockey event. If it is not, there is no guarantee the transfer will be done before the roster is due.

Do I have to affiliate with a club for training, practice, local tournaments or insurance purposes?

No, you can train and practice with any club. Affiliation has nothing to do with your Participant Accident Policy with ESIX Insurance.

When should the Club Affiliation Transfer form be used?

The transfer form should be used when you have an extenuating circumstance that hindered you from affiliating with the club during the open enrollment period.

Are both signatures on the transfer form required?

Yes.

What if my club won’t sign off on the transfer form?

If your club will not release your affiliation, the form may be sent to membership@usafieldhockey.com. The application will be reviewed and a final decision will be determined by the Executive Director.

What constitutes a guest player?

An athlete not affiliated with your club or any club.

When should I sign off on the Club Affiliation Transfer form?

When there was an extenuating circumstance that hindered the affiliated athlete from affiliating with the club during the open enrollment period.

What if I feel I should not sign off on the transfer form for one of my affiliated athletes?

You do not have to sign off on the form if for a valid reason.

Do my athletes have to affiliate with my club for training, practice, local tournaments or insurance purposes?

No, athletes can train and practice with any club. Affiliation has nothing to do with the Participant Accident Policy with ESIX Insurance.
