National Medical Network Referral Form

If you’re a Team USA athlete looking for accessible medical care – from orthopedic care to neurosurgery - the U.S. Olympic Committee encourages you to learn more about the National Medical Network.

The NMN provides medical care to athletes through partnerships with top-tier medical providers that have proven expertise in sports medicine. Providers who are invited to join the NMN must have a strong history within the following areas to qualify: multidisciplinary care, sporting event coverage, offer screening, prevention and performance services, and participation in research and education.

Medical destinations are designated by the USOC as either national or regional based on the scope of available services. National and regional providers are currently available in Colorado and New York. The USOC is striving to further develop and expand the National Medical Network to meet athletes' needs.
